The Special Needs Access Point (SNAP) is a single point of access for children up to 12 years of age who are currently attending or seeking to enroll in a licensed childcare program in Waterloo Region. Part of the Special Needs Resourcing (SNR) Collaborative, SNAP Accepts referrals from childcare programs, parents and community agencies.
The SNR Collaborative has recently launched a new website where you can find more information about referrals, what supports are available, and resources for both parents and ELCC’s. We invite you to visit the website and try the new referral system.
If you are from a child care program or other agency and want to make a referral, you can still complete the SNAP Referral and Intake Package. (to be completed together with the family). If a referral and intake package has already been completed and you are identifying additional concerns, please complete the SNAP New Service Referral Form
The LAST DAY to submit referrals with these forms is January 5, 2020. Beginning January 6, 2020 all referrals must go through the SNR Collaborative website. The last day the SNAP fax line will be in operation is January 17, 2020.
